WELCOME TO CROSSROADS DAY CENTER

Here is a list of our current Day Center Policies & Procedures.

• Pick up time starts at 8:20am *Due to other clients in the van, staff is only allowed to wait outside for 5 minutes.
• 8:35am North Orem - PG Pickup & Provo - South Orem Pickup
• Pick up time will vary based on routes, traffic, weather etc. 
• Please be ready and on the lookout for our van. 

• Drop off time starts at 2:20pm
• 2:30pm North Orem - PG Pickup & Provo - South Orem Pickup
• Drop off time will vary based on routes, traffic, etc. 
• We require someone to be at home/ facility when we drop off the client. 

• Clients are responsible to bring their own lunch.
• Please note, we do not have available lunch at the Day Center. Crossroads will provide 1 snack before lunch.

• Adult diapers are required for any client that has toileting issues. 
• Clients are required to provide their own adult diapers and cleaning wipes.

• All clients are required to bring an extra pair of clothes that will be left at the Day Center.

• Field trip fees must be paid by clients. 
• Please review the monthly calendar at www.cryouth.com for upcoming activities and fees. Most activities are free or at least $6-$10. Crossroads will invoice clients at the end of the month.

• Sick Protocol.
• Please help us keep our Day Center free from sickness, if the client is feeling sick or has a fever please keep them home, they are allowed to return to the Day Center 24 hours after being symptom free. 
• Client with a cough, please send them with a mask.

• Guardians/Caretakers are required to pick up from the Day Center or Field Trip clients if they are having behavioral or health issues. 
• We are unable to transport during day center hours. 

• We have a right to suspend or terminate services if a client becomes aggressive towards other clients or staff members. 

• Snow Delays, Guardians/Caretakers will be notified between 7:30-8:00am if there will be a 2 hour delay, if roads and weather conditions are clear by 10am we will start pickup. *Drop-off’s will also be delayed until 4pm, some clients won’t be home until 5pm. Possible field trip cancellation. Caretakers can drop off their client to Day Support if we are delayed due to weather conditions.

• Human Right & Restrictions Clients that have current HRR, please make sure we have a current copy.

• Client’s not attending? Please give us a 24 notice courtesy if the client will not be attending Day Support.
